By licensing your dog you understand and agree to the following:

• You will need to complete TWO forms (one for payment and one for sending your animal's pertinant information, which includes the rabies certificate and, if applicable, spay/neuter documentation).
• If you do not complete BOTH forms within 24 hours of each other will result in an automatic refund and no license will be issued.
• If the license purchased exceeds the expiration of the rabies vaccination a refund will be issued for the difference.
  (i.e you cannot purchase a three year license with a one year rabies certificate)
• All refunds will result in a 10% processing fee.
